Looking for a Game Over screen to be made

I have no way to make a half decent title / game over screen on my own so....could someone else do it for me?

I'll think about title later; but Game Over I know what I want it to look like.

Similar to the 2k3 RTP default screen, but less white, more red; but relatively bright; not crimson and in place of 'Game Over' have it say 'Death' or 'Failure' (Ideally, I'd have both screens made so I can see what they look like and decided, but I'm not fussy...)

Anyone willing to help, I'll trade a whole big bunch of my gratitude for it!
